Just because it starts on the 4 draw does not mean it is in third position. The root note is the 4 draw in third. A song can start anywhere, so just be careful.

Don't sweat it.
You'll get an intuitive grasp as you gain comfort level. Just find a tune or backing track to play along with. Start pulling out harps and playing along. Don't worry about whether you've grabbed an E, A, D, G or C. Just keep working it until the notes that fit arrive. Eventually, you realize that you are flowing in what feels like different "positions". These "positions" are all comprised of patterns (scales, arpeggios etc). Some require more dexterity than others (bends, blows etc).
As this comes to you, keep looking at your circle of fifths to keep track of how this is all working out. But, try not to get stuck in the rut of looking at the circle and THEN reaching for the harp because you think the A or the D should be used for the E tune. The "B" might just work out better for that E track. Try not to limit yourself before you find freedom.

Like buz said, those postions only really matter when you play with other instruments, so you can call out what key you are in, or for them to play in. Then, the Circle of 5ths(and/or thr process of elimination)come into play. You're doing a good thing as far as training your ear. All the pieces will fall into place. ----------
